鑒別診斷
對于癥狀不明顯的瘧疾，或疑似瘧疾的其他疾病，應進行鑒別。瘧疾有發熱和肝、脾腫大癥狀，應與有此特征性癥狀的其他疾病相鑒別。
1.與常見疾病鑒別
（1）血吸蟲病既往史中曾有在血吸蟲病流行區接觸過疫水和有尾蚴皮炎史。發熱和肝、脾腫大、消化道癥狀有腹瀉、黏血便等，常見嗜酸性粒細胞增多。血吸蟲病試劑盒測試，抗體、抗原均陽性。
（2）阿米巴肝膿腫不規則發熱，肝明顯腫大和有明顯壓痛，白細胞計數增多，以中性粒細胞占多數，超聲波檢查可見腫塊。
（3）敗血癥畏寒或寒戰、高熱，肝、脾腫大。可出現遷徙性膿腫，白細胞和中性粒細胞明顯增多。一般可追問出感染原因及過程。血細菌培養陽性。
（4）傷寒初為弛張熱，后為稽留熱或弛張熱，出現玫瑰疹，可見胃腸道癥狀和全身中毒癥狀。血、骨髓、糞尿細菌培養陽性，肥達氏反應陽性。
（5）鉤端螺旋體病弛張熱或持續性發熱，有腓腸肌痛的特征性癥狀。可能出現皮膚黏膜出血，肝、脾腫大。血清免疫學試驗陽性。
（6）急性腎盂腎炎不規則發熱，腰酸，尿頻或尿痛。尿中出現紅、白細胞和蛋白，尿液細菌培養陽性。
（7）布氏桿菌病弛張熱、搞忘炎是特征性癥狀之一，脾腫大而有壓痛。血清凝集試驗或ELISA試驗陽性。
（8）病毒感染如病毒姓感冒，發熱、畏寒，常伴有明顯的上呼吸道感染癥狀。又如登革熱，高熱伴畏寒，肝、脾腫大、四肢及軀干疹。有些病毒感染，不規則發熱，多方面檢查均未查出病因，用分子生物學技術檢測，卻測出一種病毒的陽性結果。
2.與腦性瘧以外的昏迷的鑒別
腦膜炎、腦炎、癲癇、腦膿腫、腦瘤、腦血管意外、熱帶地區的錐蟲病，鐮狀細胞病等均可引起昏迷，如將其臨床表現與實驗室診斷結果綜合起來分析，不難判斷是否腦型瘧引起的昏迷。不過，至少在現階段，凡近期在非洲和東南亞等瘧疾流行地區居留過而出現昏迷癥狀者，腦性瘧是首先要考慮的疾病。
